Assessment of Immunization level. Immunizations given for Polio, Measles, Rubella, Tetanus, Diphtheria, Pertussis, Mumps, Hepatitis B, HIB, Varicella, Flu, Pneumonia.

Eligibility/Fees
Pediatric Immunization: For all children under the age of 18, parent or guardian consent required. Fees charged. Walk-in services at Family Care Centers. Low fee-maximum set per family. No one is denied service due to inability to pay.
Adult Immunizations:
Set fees charged. If job related, payment is due at time of service unless contract, voucher, or other arrangements for billing have been made.
