Treatment, education and writing of prescriptions for acute and chronic illnesses, including the flu, a cold, diabetes, exams, immunizations.
Women's health exams, including breast exam, pap and pelvic exam with appropriate laboratory services to screen for cancer.
If a problem cannot be resolved at our Family Care Center, a referral may be made to a hospital or a specialist.
Referrals:
a) Specialist and sub-specialist for definitive diagnosis and prescription.
b) For hospitalization as needed
c) Public Health Nurse or other appropriate agency.

Smoking, obesity, hypertension, diabetes, self-exams, other life style behaviors
Eligibility/Fees
Everyone is eligible for Primary Care Services. Medi-Cal, CHDP Treatment, and private insurance are accepted. Non-Medi-Cal patients are screened financially for Medi-Cal, and MISP eligibility. Ability to pay program (sliding fee) is available for low income patients who provide proof of income.
